SBAR handover note
Situation, background, assessment, recommendation. Empty fields stay empty.
What this tool does
The handover note builds an SBAR from four boxes: situation, background, assessment, and recommendation. The rules text prints each label and, if you left a box blank, the sentence “Not provided. Do not fill this in from memory here.” It then states that the note repeats what you typed and does not suggest a dose, a diagnosis, or a treatment. The score is 25 times the number of boxes you filled, so four filled boxes score 100 and an empty form scores 0. A model, when configured, is still required to stay inside what you typed, and the page tells you to review it. Do not put a patient identifier into a practice box you do not need. This is a rehearsal of the shape of a handover.
How to use it
Fill only the facts you would actually say at the bedside or on the phone. Leave unknown fields blank and watch the note refuse to invent them. Read it back. If a model added a drug or a number, delete it. The dosage calculator is the separate place for a division you already have on an order.
Worked example
Situation “Pain reported at the surgical site”, background left blank, assessment “awake and speaking”, recommendation “review by the person who holds the order”. The rules note prints those three lines and, for background, “Not provided.” It does not add a pain score, a drug, or a dose. Filling the blank yourself is the next act, from the chart, not from this page. The jobs link on this page searches HiringBuddy for published roles matching “nurse”. An empty list means nothing published matched. It is not a comment on the wider market. A signed-in candidate can save the attempt on their profile. Saving is not an application, and it does not show the attempt to an employer. Employers see people who apply to their own jobs.
Questions
- Will it complete a blank SBAR field?
- The rules path will not. A blank stays marked as not provided.
- Can it suggest a medicine?
- It should not. The closing line says it does not suggest a dose, a diagnosis, or a treatment.
- What is the score?
- 25 points for each of the four boxes you actually filled.
